Please click the link to access the Healthie Portal and complete the New Patient Intake Paperwork. Kindly ensure that all required documents are submitted at least 48 hours before your appointment to prevent appointment delays or rescheduling.

Please select your appointment time thoughtfully. Appointments canceled or rescheduled with less than 48 hours' notice will result in an $80 fee. Scheduling times are limited due to the personalized attention provided to each patient. Failure to provide adequate notice restricts appointment availability for other patients and impacts the operation of FFTN. We appreciate your understanding and cooperation.
